Once you register, you can log in at any time to:

- Complete patient forms
- Request prescription refills
- Request and view your medical records and test results
- Pay your bill
- Complete our patient satisfaction survey
- Communicate with our office
If you are new to our Patient Portal
- If you have not received authorization to begin using the portal, call our office to obtain a registration PIN.
- Click on the Register link above.
- Follow the directions to create a new account. You will create your user ID and password.
- Once approved by the practice, you can log in and begin using our convenient online services by clicking on the appropriate sections in the dashboard.
